Description:The PRISM Performance Diagnostic Tool determines the overall level of RHIS performance, i.e., the level of data quality and use of information. It also captures technical and organizational determinants, such as indicator definitions and reporting guidelines; the level of complexity of data collection tools and reporting forms; and the existence of supervision and feedback mechanisms. The Performance Diagnostic Tool is for use at health facility and district or higher levels.
